Position

Hospice Registered Nurse RN | PRN/Casual | Mankato, MN area.

Schedule

Casual / PRN / Pick up shifts as they become available. Shift: 8:00am-4:30pm. Minimum one weekend per month, on-call for one weekend a month and on-call for one holiday per year.

Pay

Targeted pay range: $42.00 - $46.00 per hour. Pay determined based on experience, qualifications, certifications, licenses, and education.

Essential

Job Responsibilities

Receive and respond to phone calls during assigned shifts, make visits to patients with changing conditions, and complete weekend admissions.

Complete an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs, providing a complete physical assessment and history of current and previous illness.

Assess and evaluate patient status by writing and initiating plan of care, regularly re‑evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary.

Develop a care plan establishing goals, based on nursing diagnosis and incorporating palliative nursing actions, including the patient and family in the planning process.

Administer medications and treatments as prescribed by the physician in the physician's plan of care; record pain and/or symptom management changes/outcomes as appropriate.

Initiate the plan of care and make necessary revisions as patient status and needs change; communicate with the physician regarding patient needs and report changes in the patient's condition; seek physicians' orders as required.

Provide health care instructions to the patient as appropriate per assessment and plan.

Driving Requirements

Local travel and driver's license required.

Minimum

Required Qualifications

Graduate of an accredited Registered Nursing program.
1 year of nursing experience in post‑acute care setting.
Licensed as a Registered Nurse within the state in which working and maintain licensure.
Valid driver's license, proof of insurance, satisfactory motor vehicle record.
Ability to communicate effectively both verbal and written.
Proficient with Microsoft Office Suite.
Ability to follow all safety rules, regulations, policies and procedures of the facility and applicable laws.

Preferred Qualifications

Bachelor of Science degree in Nursing.
One year nursing experience in Hospice.
