Position Summary
Under general supervision, the Caring for Two (CFT) Recovery Coach reports to the Outreach and Care Coordinator/PPI Supervisor. The Recovery Coach/Peer Support Specialist provides outreach, education, peer support, and care coordination/navigation services for pregnant and postpartum women (with a child up to 18 months) who have a past history or are at risk of substance use and their families. The role requires extensive travel throughout the greater Austin area and surrounding counties, including occasional evening and weekend hours as needed.
Responsibilities- Provides outreach, education, peer support, navigation, and care coordination services for families with a pregnant or postpartum member at risk for substance use.
- Implements a strengths‑based assessment to evaluate participants’ strengths, challenges, and goals across multiple life domains.
- Utilizes person‑centered planning principles to develop recovery plans and engages participants regularly to monitor progress and address barriers.
- Conducts face‑to‑face and telephone coaching, develops individualized recovery plans, coordinates resource referrals/linkages, and leads recovery support and peer support groups.
- Facilitates family bonding activities, recovery wellness activities, and community integration initiatives.
- Serves as liaison with Children’s Protective Services (CPS), the medical community, and other systems of care, including coordination of Plans of Safe Care.
- Provides ongoing care coordination, advocacy, and mentoring to ensure participants maintain recovery.
- Engages and retains participants, motivating healthy behavioral changes and managing risks of disengagement.
- Conducts scheduled recovery check‑ups at 5‑day and 3‑, 6‑, 9‑, and 12‑month intervals via phone, face‑to‑face, or internet.
- Participates in street outreach, overdose prevention, and other harm‑reduction activities as required.
- Supports training, quality‑improvement initiatives, data collection, and documentation required by Texas Department of State Health Services (DSHS) and other funders.
- Maintains accurate records, completes required documentation, and reports information professionally and accurately.
- Promotes recovery language and practices, including motivational interviewing and recovery‑oriented language.
- High school diploma or GED required.
- Two years of related experience and/or training working with women at risk of substance use or with co‑occurring disorders preferred.
- A minimum of 500 hours of directly related experience (including 250 supervised hours) within the past 1–3 months required.
- Previous experience as a Peer Recovery Support Specialist, Recovery Coach, Community Health Worker, or Patient Navigator preferred.
- Solid knowledge of community resources, recovery community, and documentation per licensure and funder standards.
- Knowledge of substance use and ethical standards required.
- Current Peer Recovery Coach / PRSS designation from the Department of State and Health Services, including completion of state‑approved training and recertification every two years.
- Peer Recovery Support Specialist certification with the Texas Certification Board of Addiction Professionals (TCBAP) or an equivalent credential.
- Approval from the Texas Department of Criminal Justice (TDCJ) is required for work in TDCJ‑funded programs, if applicable.
- At least one year of lived recovery experience as required by the Department of State and Health Services.
- Active participation in a stable personal recovery support group at the time of hiring.
