Impact Health PC, a leading advanced practitioner group in skilled nursing facilities across the Northeast, that is dedicated to prioritizing patient care. We specialize in Medical, Psychiatric Services, Wound Care and Palliative Care.

Compensation and Benefits:
Earning potential: $35,000
Malpractice insurance provided
License reimbursement and CEU support
Paid training and onboarding
Ongoing education and clinical support, including physician-level consult access
Autonomy with Support to deliver independent care with access to physician-level consults.
Key Responsibilities Deliver person-centered care using evidence-based practices
Manage caseload and fulfill psychiatric consultation needs of skilled nursing facility
Provide assessment, diagnostic, and treatment services
Provide psychoeducation and medication education to patients and/or families
Collaborate with interdisciplinary facility teams
Complete timely documentation (within 24 hours)
Ensure compliance with state regulations in SNF settings
Qualifications:
Licensed Psychiatric Nurse Practitioner in the state of practice.
Experience in skilled nursing facilities, hospitals, residential care, geriatric services, and outpatient psychiatric settings preferred.
Minimum of 2 years of clinical experience providing mental health care to adult populations.
Proficient with EHRs and clinical technology
Strong communicator with a collaborative mindset
