• Active and unrestricted Physical Therapist (PT) license in the state of Ohio required
• Graduate of an accredited Physical Therapist program
• Previous Physical Therapy experience preferred
• Home Health experience preferred
• Ability to evaluate patients and develop individualized plans of care
• Strong clinical assessment, communication, and documentation skills
• Ability to work independently in a home-based care environment
• Valid driver's license and reliable transportation required
• Ability to travel locally to patient homes throughout the assigned service area
• Ability to perform required physical job duties, including assisting with patient mobility and transfers
• Ability to pass background screening and onboarding requirements

• Evaluate patients to determine functional limitations and appropriate physical therapy needs
• Develop and implement individualized physical therapy plans of care
• Provide therapeutic exercise, functional training, mobility and gait training, manual therapy, and other appropriate physical therapy interventions
• Evaluate patient response to treatment and modify treatment plans as clinically appropriate
• Establish goals designed to improve mobility, strength, balance, function, and independence
• Educate patients, families, and caregivers on treatment plans, exercises, safety, and home care techniques
• Complete required evaluations, reassessments, progress notes, discharge documentation, and other clinical records accurately and on time
• Communicate patient progress and changes in condition with physicians and members of the interdisciplinary care team
• Coordinate with and provide appropriate direction to Physical Therapist Assistants (PTAs) involved in the patient's plan of care
• Promote patient safety, independence, and quality of life in the home environment
• Follow agency policies, clinical standards, HIPAA requirements, and applicable regulatory guidelines

Job Description Wise Home Care is seeking an experienced Physical Therapist (PT) for PRN Home Health opportunities throughout Richland County, Ohio. The Physical Therapist will provide individualized therapy services to patients in their homes, focusing on improving mobility, restoring function, reducing pain, increasing independence, and helping patients safely perform activities of daily living. Responsibilities include completing patient evaluations, developing and updating individualized plans of care, providing skilled physical therapy treatments, educating patients and caregivers, coordinating with physicians and the interdisciplinary healthcare team, and maintaining accurate and timely clinical documentation.

This position is ideal for a Physical Therapist who enjoys working independently, providing one‑on‑one patient care, and helping patients achieve meaningful functional improvements within their own homes.
