Home Health Physical Therapist

• Plan and administer prescribed physical therapy treatment for patients in their homes
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals
• Develop and implement rehabilitation programs consistent with physicians' Plans of Treatment and patient-team goals
• Provide PTAs and Home Health Aides with written care instructions
• Oversee PTA and Home Health Aide performance and documentation
• Document patient observations, interventions, and evaluations promptly and thoroughly
• Report patient progress to physicians, Clinical Manager, care team, patients, and families
• Submit required evaluations, plans, progress notes, and discharge summaries
• Help patients regain strength, mobility, and independence through personalized home healthcare

• Current and unrestricted Physical Therapy license
• Current CPR certification
• Experience collaborating with a team of healthcare professionals
• Valid state driver's license
• Personal vehicle liability insurance in accordance with residing state minimum required limits or specified higher limits
• One year of experience as a Physical Therapist (preferred)

Demonstrates expertise in planning and administering physical therapy treatments, developing rehabilitation programs, and collaborating with healthcare teams to enhance patient outcomes. Holds a current Physical Therapy license and CPR certification, ensuring compliance with healthcare standards.
