Home Health Aide – PRN

• Provide direct patient care under the direction of the RN and according to the Aide Plan of Care
• Assist patients with self-administered medications as permitted
• Measure temperature, pulse, and blood pressure and report abnormal results
• Provide or supervise personal hygiene, grooming, skin care, oral care, and nail/foot care
• Assist with feeding, nutrition, positioning, and recording food and fluid intake
• Assist with toileting, bed pans/urinals, catheter bags, and ostomy procedures
• Assist with mobility, exercises, positioning, transfers, and ambulation
• Report changes and document patient information and care rendered
• Practice infection control principles and maintain a clean, safe, comfortable environment
• Perform light household tasks, laundry, bed making, meal preparation, housekeeping, and shopping when authorized
• Collaborate with patients, caregivers, doctors, supervisors, and other staff to ensure continuity of care

• Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months
• Must meet applicable state certification requirements
• A valid driver's license, auto insurance, and reliable transportation are required
• Must be in good standing on the HHA Registry (if applicable)
• Must have completed HHA/CNA course to work for a Medicare certified agency
• Minimum one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency
• Must complete TB screening if selected
• Must maintain personal vehicle liability insurance in accordance with applicable requirements

Demonstrates expertise in providing direct patient care, including assistance with self-administered medications, personal hygiene, and mobility support. Maintains compliance with state certification requirements and infection control principles while ensuring effective communication with patients and healthcare teams.
