JOB TITLE:
PDN / Private Duty Nurse (LPN or RN) - Full-time Overnight Shifts
Active Ohio LPN or RN in good standing is required.
At least 1 year of paid experience at a home health agency (or equivalent) is required; 2-5 years' experience is preferred.
1+ years of experience in the following is highly desirable:
Case Management, wound care, PICC/PORT/IV Care, lab draws, plus OASIS and Non-OASIS assessments. Nurse Supervision experience is a plus.
- Night Shift (9 PM - 5 AM), 7 days per week, all overnight hours (56 hours/week)
- Client is willing to split shifts between 2 PDNs if necessary
- Qualified applicants may reside in or near any primary service area so long as travel requirements are fulfilled throughout the assigned region.
- FT = Mon-Sun, 9pm - 5AM (56 hrs/wk incl. 16 hrs OT)
- PT = partial of schedule above (No OT)
- No On-Call shifts required.

About the RoleWe are seeking a highly skilled, experienced, accountable, and self‑motivated home health LPN or RN to join our agency as a Nighttime Home Health Private Duty Nurse / PDN (RN or LPN) to care for clients in and around Lexington, OH (Richland County).
- This role may be filled as Part-Time only or start as Part‑time and quickly build up to Full‑time, with the option to add additional clients in surrounding areas for additional hours if desired (not required).
- LPNs: $28 - $30 per hour
- RNs: $35 - $40 per hour
Compensation is based on employee qualifications, certifications, experience, skill specializations, positive employment history, and overall client & team fit.

- Graduate of an accredited school of professional nursing.
- Active LPN or RN, with current license to practice as a Licensed Practical Nurse/Registered Nurse in the state of Ohio.
- At least 1 year in the home health industry + 1 year of nursing experience. Additional home health nursing experience/familiarity is highly desired.
- Active CPR and First Aid certificates highly desired (agency will provide recertification if required).
- Minimum of 1 year direct patient experience with ventilator care is required.
- Good interpersonal communication skills (written and verbal).
- Desirable character traits: reliable, honest, professional, compassionate, detail‑oriented (in care + documentation), accountable, friendly and accommodating, respectful.
- Current driver's license or state , plus reliable transportation, are required.
- Excellent attendance record with past employers.
- Ability to meet all of the basic job requirements is a must.
- Willingness and ability to pass state and federal background check.
A successful PDN at Home Care Network, Inc. performs skilled nursing functions in accordance with federal, state, and local laws and within the guidelines of the professional organization and agency.
