Employment Type- Long‑term employment for Full‑time or Part‑time positions*

Schedule- Night shift: 9 PM – 5 AM, 7 days per week (56 hours per week)
- Potential split shifts with two PDNs if necessary
- Travel to clients in and around Lexington, OH (Richland County); travel radius customized per employee
- LPNs: $28–$30 per hour
- RNs: $35–$40 per hour
- Weekly pay (Fridays) including onboarding, training, and an additional 4 hours on the first paycheck
- Flexible, customizable schedules; backup accommodations for vacations

- Graduate of an accredited nursing school
- Active Ohio LPN or RN license in good standing
- At least 1 year in home health industry + 1 year of nursing experience; additional home health nursing experience highly desired
- At least 1 year direct patient experience with ventilator care
- Active CPR and First Aid certificates desirable; agency provides recertification when required
- Good written and verbal communication skills
- Reliable, honest, professional, compassionate, detail‑oriented, accountable, friendly, and respectful
- Current driver’s license or state reliable transportation
- Excellent attendance record with previous employers
- Willingness to pass state and federal background check
- Ability to meet all basic job requirements
A successful PDN performs skilled nursing functions under the direction of the RN Case Manager, following agency policies and the state Nurse Practice Act. LPN/RN delivers care to clients as delegated by the RN Case Manager.
Essential Functions/Areas of Accountability- Provide professional nursing care under direction of a registered nurse, communicating status changes per agency guidelines
- Conduct skilled nursing visits per RN Case Manager’s directions, following the care plan
- Recognize and report changes in client condition to Case Manager and physician
- Communicate information promptly, verbally and in documentation
- Perform clinical tasks within scope of practice per policies (personal care, assessment, medication monitoring, client teaching, emergency intervention, venipuncture when authorized)
- Promote personal safety and a safe environment for clients and coworkers
- Demonstrate knowledge of safety/infection control practices by complying with policies and procedures
- Make recommendations for safety improvements
- Maintain confidentiality and respect for client and staff identities
- Participate in quality improvement program activities
- Perform other related duties as needed
- Frequent standing, walking, bending, stooping, occasional sitting
- Weight handling: push/pull 10–20 lbs occasionally, 20–30 lbs infrequently; carry/lift 5–10 lbs occasionally, 10–30 lbs infrequently
