Role: RN Certified Diabetic Care and Education Specialist
Responsibilities- Assess patients' medical, psychosocial, socioeconomic, educational, and activity needs to tailor diabetes management plans
- Facilitate group education sessions following National Standards for Diabetes Self-Management Education, monitor blood sugar logs, and assist with problem-solving
- Provide individual counseling to set goals and develop action plans for diabetes self-management
- Teach finger stick and alternate site blood glucose testing techniques
- Instruct on various insulin administration methods, including syringes, pens, and devices for special needs
- Maintain thorough knowledge of management therapies for Type 1, Type 2, and gestational diabetes
- Ensure continuity of care by evaluating patient progress and updating management strategies
- Stay current with diabetes research and update educational materials accordingly
- Graduate from an accredited nursing school
- Certified Diabetic Care and Education Specialist (CDCES)
- Licensed as a Registered Nurse in Texas
This role involves providing comprehensive diabetes education and management support, utilizing current research, and ensuring personalized patient care to improve health outcomes.
