Position Summary:Case Manager (MLTSS/D-SNP Programs)
The Case Manager is responsible for developing and implementing healthcare strategies to ensure quality care for members, particularly those in Managed Long-Term Services and Supports (MLTSS) and Dual Special Needs Programs (D-SNP/FIDE).
Key Duties- Conducting in-home assessments using comprehensive tools.
- Creating and managing individualized care plans.
- Coordinating services such as adult/pediatric daycare, personal care, and nursing facility placements.
- Collaborating with members, families, providers, and interdisciplinary teams.
- Ensuring cost-effective service delivery and accurate documentation.
- Mentoring new hires once proficient.
- Traveling 50-75% within assigned counties.
Advanced clinical judgment, critical thinking, strong communication, and assessment skills. NJ Choice Certification is required.
Required Qualifications- Active and Unrestricted RN license in NJ required.
- Must reside in Somerset County NJ
- 3+ years clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care required.
- Successful completion of the NJ Choice certification is a contingency of employment. As per NJ Division of Aging Services, Office of Community Choice Options guidance, an exam score of 80% or higher is required, along with successful completion of the State training modules, and field mentoring component.
- Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed per our company expense reimbursement policy
- Case management and/or discharge planning experience preferred.
- Managed Care experience preferred
- Crisis intervention skills preferred
- Associate degree equivalent or higher
