RN Case Manager

THIS IS AN HOURLY POSITION, ESTIMATED SALARY BASED ON HOURLY PAY RATES

Pay Rate: $42–$45 per hour | Estimated Yearly Salary: $87,k/yr |

Work Schedule:

Monday-Friday 32-40 hours (M-F for 40 hours or Tues-Fri for 32 hours)

• No OASIS work!
• Provides care as defined within the scope of the state’s nurse practice act.
• Makes the initial evaluation visit and regularly reevaluates the client’s nursing needs.
• Prepares clinical and progress notes and summaries of care.
• Serve as backup for Critical Care Clients, In‑School Staffing & PDN
• Assures proper maintenance of clinical records in compliance with local, state, and federal laws.
• Helps educate and coach care-giving staff and assist with new hire clinical training, as needed.

• Current unrestricted RN license in Minnesota
• Minimum two (2) years of clinical experience with one (1) year of experience in a home care agency, primary care clinic, or health facility, preferred.
• Active CPR Certification
• Negative TB skin test or Quanti
  
  FERON‑TB Gold (QFT) blood test (completed within the last 90 days)
• Reliable car & Auto Insurance
• Valid Driver's License
• Clean background and Criminal Record
• Understands regulatory compliance with state and federal regulations.
• Ability to work on call rotation with compensation
