Transitional Care LPN – Adults and Pediatrics

• Provide adequate staffing up to 24/7 for various situations.
• Take‑home team to say yes to new referrals.
• Fill staffing gaps to meet the needs of existing clients.
• Assist with growing our field team by training new nurses.

• LPN with a current valid nursing license.
• Commit to working agreed‑upon shifts.
• Willingness to work in all assigned homes.
• Willingness to train to the top of your license (infant, pediatric, adult, trach and/or vent, and IVs).
• Willingness to precept onboarding nurses.
• Commitment to attend school with clients if applicable.
