- Develop and initiate individualized care plans, ensuring each patient is set up for safe and effective home health services.
- Identify risks, gaps, and service needs early, coordinating the right disciplines and resources from the start.
- Communicate clearly with providers, caregivers, and internal teams to ensure alignment and continuity of care.
- Set the tone for quality, compliance, and patient experience from the very first visit.
- Active Minnesota RN license with a strong clinical foundation.
- Proven experience with home health admissions and OASIS documentation.
- Ability to think critically, prioritize quickly, and make confident decisions independently.
- Strong communication skills focused on patient education and care coordination.
- Reliable transportation and comfort working in a field‑based environment.
- Mileage reimbursement provided.
- Territory‑based scheduling across the Twin Cities Metro.
