Job Title: Wound Care Nurse Practitioner (AGNP/CNP) | On-Site Senior Care
Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

We're seeking a full-time Wound Care Nurse Practitioner who enjoys identifying concerns early, intervening before complications occur, and helping patients remain healthier where they live. Our providers deliver care directly inside assisted living communities, memory care settings, transitional care programs, and long-term care communities throughout the Twin Cities Metro. Rather than waiting for patients to navigate the healthcare system, you'll bring specialized wound care directly to them, helping improve outcomes while reducing avoidable hospitalizations and complications.

What You'll Be Doing
- Provide advanced wound care assessments, treatment planning, and ongoing management within senior living communities.
- Conduct routine rounding visits and follow-up care across assigned communities.
- Manage medications, diagnostics, treatment plans, and clinical interventions.
- Collaborate with physicians, nurses, therapists, caregivers, and community staff.
- Identify risks early and implement strategies that help prevent complications and hospitalizations.
- Support continuity of care while improving healing outcomes and quality of life.
Qualifications
- Active Minnesota Certified Nurse Practitioner (CNP) license.
- Experience in wound care, geriatrics, primary care, post-acute care, skilled nursing, or related settings preferred.
- Strong clinical judgment and confidence managing medically complex patients.
- Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
