Job Overview
Fairview has an immediate opening for a Licensed Practical Nurse to join our Specialty team in Maplewood, MN. This is a 0.01 FTE casual on‑call position, non‑benefits‑eligible opening. Clinic hours are Monday‑Friday 8am‑5pm with no weekends. The position requires flexibility to float to all three regions:
East (Maplewood, Woodbury, Wyoming), Central (Minneapolis, Maple Grove), and South (Edina).
The LPN performs delegated nursing and medical functions of data collection and delivery of care to an assigned group of patients or an individual patient. The LPN works under the direction of a Registered Nurse, physician, or authorized practitioner and in collaboration with other members of the health‑care team. The LPN is responsible for adhering to all policies, procedures, and nursing practice guidelines.
Responsibilities- Collect data for patient assessments.
- Involve the patient, family, and other health team members in gathering relevant data.
- Document data collected in the medical record.
- Report unusual data or changes in patient condition to the RN, or physician/authorized practitioner in a timely manner.
- Deliver care as delegated by the RN or physician/authorized practitioner to the assigned patient population, or individual patient.
- Follow the patient’s plan of care.
- Prioritize work based on patient acuity, available resources, and patient/family preferences, in collaboration with the RN or physician/authorized practitioner.
- Perform high and low risk technical procedures, including medications, as defined by the clinical setting.
- Recognize when different approaches may be needed based on age or cultural differences.
- Implement assigned aspects of the patient teaching plan.
- Document care in the medical record.
- Effectively communicate compliance and concerns to delegating RN or physician/authorized practitioner.
- Takes action to assure continuity of care for all assigned patients.
- Adhere to all relevant policies, procedures, and nursing practice guidelines.
- Respond appropriately to emergent situations.
- Communicate effectively with other members of the health‑care team.
- Communicate verbal and written information in a clear, concise, accurate, and timely manner.
- Use communication approaches appropriate to patient/family needs.
- Vocational School diploma or Associates Degree in Nursing.
- Minnesota Licensed Practical Nurse (LPN).
- 1 year of ambulatory care experience.
- Previous wound care experience.
- Basic Life Support certification (American Heart Association or Red Cross).
