Job Title: Hospice RN Float – Admissions, Liaison & Clinical Coverage (RN)
Pay: $90,000 – $130,000 DOE plus Full Benefits Package
Schedule: Monday – Friday with on‑call and weekend rotation
Location: Greater Twin Cities Metro Area, 21‑County Service Area (territories assigned based on coverage needs)

We are hiring a Hospice RN Float to support admissions, referral activity, field visits, and clinical coverage
, playing a key role in both patient care and organizational growth.

- Review referral pipelines, intake updates, and reports to identify hospice‑appropriate patients.
- Conduct informational visits with hospitals, SNFs, and senior living communities.
- Partner closely with intake to prioritize referrals and coordinate timely admissions.
- Complete hospice admissions including eligibility, education, consents, and start of care.
- Provide visit nurse coverage for routine, PRN, and urgent patient needs.
- Collaborate with discharge planners, social workers, and providers on care transitions.
- Support census growth through consistent follow‑up and strong referral relationships.
- Maintain accurate EMR documentation and ensure compliance with hospice regulations.
- Step into clinical gaps as needed to ensure seamless patient care delivery.
- Hospice experience preferred or strong background in ICU, oncology, palliative care, or high‑acuity settings.
- Experience with admissions or care transitions strongly preferred.
- Comfortable working independently in a field‑based role.
- Strong communication and relationship‑building skills.
- Highly organized, flexible, and able to adapt quickly.
- Reliable transportation and ability to travel throughout the service area.
