Promotor(a) de Salud/Bilingual Community Support Specialist Human Services

A full-time Promotor(a) de Salud/Bilingual Community Support Specialist position is available in the Human Services Department. This position performs intermediate county services work serving as a trusted liaison between the Hispanic community, partner organizations, and County departments. The position promotes equitable access to health and human services through culturally responsive outreach, education, advocacy, resource navigation, and care coordination. Work involves building relationships with Spanish-speaking individuals and families, connecting them to appropriate services and resources, collaborating with County departments and community partners to reduce barriers to access, and supporting improved health and well-being.

Work is performed under the general supervision of the assigned supervisor.

Essential Functions

  • Conduct outreach and build effective relationships with individuals, families, and community partners to increase access to services and promote community well-being of the Hispanic community.
  • Assess Spanish-speaking client and community needs, identify barriers to accessing services, and connect individuals with appropriate county, community, healthcare, and public assistance resources.
  • Provides translation and interpretation services, verbal and written, to support effective communication across all County departments, ensuring equitable access to County programs, services, and information for individuals with limited English proficiency and other language access needs.
  • Provide care coordination, resource navigation, referrals, and follow-up to support successful access to services and improve outcomes.
  • Deliver culturally responsive education, information, and support related to health, wellness, human services, and available community resources.
  • Advocate for equitable access to services, empower individuals to navigate systems independently, and support informed decision-making.
  • Collaborate with internal staff, healthcare providers, collaborative partners, schools, nonprofit organizations, government agencies, and other community partners to coordinate services and address community needs.
  • Assist Public Health & Human Services with interpreting services for in-home and office visits including MN Choices Assessments, immunizations, lactation appointments, scheduling appointments, and assisting individuals with financial assistance applications.
  • Assist Child Services unit with facilitating supervised visitation and transportation, provide culturally and linguistically appropriate education.
  • Participate in community engagement, program development, and interdisciplinary initiatives that strengthen service delivery and address emerging issues.
  • Track outreach activities and program outcomes. Maintain accurate documentation, records, reports, and data in accordance with applicable laws, regulations, and organizational policies, including confidentiality requirements.
  • Travel throughout the county and work in a variety of settings, including county offices, client homes, healthcare facilities, schools, and community locations, to provide services and support including interpreting services.
  • Comply with all applicable federal, state, and local laws, regulations, policies, and departmental procedures governing the delivery of human services.
  • Provide services in a professional, culturally responsive, impartial, and ethical manner, including accurate communication and interpretation when applicable.
  • Identify, document, and report suspected fraud, abuse, misuse of benefits, or other program integrity concerns in accordance with departmental policies and procedures.
  • Performs the physical and mental demands of the position.
  • Attends work during regularly scheduled hours including occasional evening or weekends hours for events.
