Job Summary
Phoenix of Maple Heights, a family‑owned skilled nursing facility, is hiring a Licensed Practical Nurse (LPN) to provide essential care in a long‑term care setting. As an LPN, you’ll provide direct nursing care, support nursing assistants, and maintain high‑quality resident outcomes.

- Provide essential care in a skilled nursing facility.
- Administer medications and treatments, ensuring safety compliance.
- Document patient conditions and update care plans accordingly.
- Support and supervise STNAs to ensure quality long‑term care.
- Monitor resident health and escalate concerns as needed.
- Hourly pay: $32/hour
- Shift differential: +$2/hour from 10 p.m.–6 a.m.
- Full‑time and part‑time available
- Valid LPN license in the state of Ohio
- BLS/CPR certification
- Experience in skilled nursing or long‑term care preferred
- Strong electronic medical record (EMR) skills
- Excellent communication and patient care skills
