Nurse Practitioner​/Physician Assistant - Primary Care; Group Home Population

Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing in Group Homes in the northeast metro of Maplewood, MN.

• Provide on-site, team-oriented primary care to patients within group home setting living communities.
• Part-time providers care for patients weekdays; tailored to part-time desires and needs.
• Manage a panel of patients autonomously seeing approximately 8-14 patients per day.
• Ability to design and manage your schedule.
• Develop and implement care plans appropriate to patients and their desires.
• Clinical support from clinical assistants and care coordination through LPNs, RNs and social workers.
• Collaborate with community partners within home care, hospice, and other resources as needed.

• Graduate of NP program or PA program from an accredited school.
• Must have and maintain a valid Minnesota License.
• Professional clinical experience in primary care.
• Proven ability to communicate effectively with strong verbal skills.
• Excellent interpersonal and customer service skills.
• Must have a valid driver’s license and minimum state required liability auto insurance; local travel is required.
• Demonstrated compatibility with Bluestone’s mission and practicing philosophies.
• Travel throughout Twin Cities Metro required.

• 2+ years of clinical experience and/or group home experience in another role.

• Compensation Base + Quality/Productivity: $115-140k.
