About the Role

An Advanced Practice Practitioner (APP) – Market Floater is a licensed and board‑certified nurse practitioner or physician’s assistant specifically assigned to Longevity Health Plan members for executing the Institutional Special Needs Plan Model of Care. APPs are not permanently assigned to one facility but float within a state market to fill vacancies, absences, and other needs. Schedule of travel and frequency vary by market and are mutually agreed to by MDCS and the new hire.

APPs requiring regulatory oversight will be hired based on available collaborative practice physicians and oversight processes in each state.

Key Tasks And Responsibilities
  • Provides collaborative primary care and services to a panel of Longevity Health Plan Services, treating in the most appropriate setting based on the member’s goals of care.
  • Executes and delivers Longevity Health Plan Model of Care activities, develops and implements advanced care planning, and embraces the treating‑in‑place model.
  • Facilitates and administers Health Plan clinical operations functions—prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, and promotes membership growth.
  • Serves as a trusted advisor to members, loved ones, nursing home staff, and member advocates; educates care teams on member needs and early identification of changes in condition.
  • Delivers effective and frequent communication and coordination with members’ PCPs, loved ones, and care team members regarding care needs, goals of care, and treatment plans or changes.
  • Supports overall market goals and targets and balances member caseload demands with other team member support needs.
  • Promotes Longevity Health Plan with members, loved ones, nursing facilities, and other stakeholders.
  • Functions autonomously and is accountable for practice.
  • Establishes meaningful relationships and effectively functions in the nursing home environment.
  • Is member‑centric and customer‑service oriented, establishing relationships that ensure effective, high‑quality care for Longevity Health Plan members.
  • Performs other related duties as assigned.
Requirements
  • Minimum of 6 months experience as a Nurse Practitioner or Physician’s Assistant with geriatric/hospice or palliative care experience, or a licensed and board‑certified new graduate NP/PA supervised by an experienced NP/PA in the LHP New Graduate Fellowship program for 6 months and completion of all competencies for independent practice.
  • Experience working in the nursing home environment is strongly desired.
  • Ability to travel statewide as directed and agreed.
  • Computer and technology proficiency required.
  • Excellent communication and people skills with physicians, patients, and family members.
  • Self‑directed but functions well in a team environment.
  • Excellent clinical skills—assessment, diagnosis, treatment, prescribing specifically for the chronically ill population.
  • Can function independently, demonstrated critical thinking, organization, and prioritization skills.
  • Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program or Master’s Degree from a nationally accredited program.
Credentials & Coverage
  • State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC, or AACN) as an Adult, Family, Geriatric, or Primary Care nurse practitioner required; or state unrestricted licensed Physician’s Assistant NCCPA board‑certified with nursing home/geriatric or hospice/palliative experience.
  • DEA license required.
  • Active national provider number (NPI) and active Medicare provider number.
  • Valid state driver’s license with a good driving record.
  • Auto insurance coverage as required by your state of residence; new employees must provide proof of coverage prior to hire and on an ongoing basis.
