RN Home Health
The RN Home Health is responsible for providing an initial, resumption, and recertification evaluation of the home health patient in their home setting, along with other routine home health visits. This includes age appropriate interventions that meet physical, emotional, developmental, psychosocial, and educational needs. The RN Home Health is responsible for the coordination of initial disciplines to begin care, including the planning of care throughout the patient episode, collaborating with the physician, patient, family member and team manager to develop the expected patient visits, scheduling the visits to the appropriate team member, incorporating therapeutic, preventative and rehabilitative nursing actions.
The RN Home Health communicates with all team members including RNs, LPNs, Therapists, Social Workers, Home Health Aides, the Physician, patient, family, and other internal or external individuals to ensure that the patient's plan of care is appropriately developed.
