Wellstar Health System is seeking an RN Case Manager to provide and supervise primary care to 25-35 patients in their homes. You will coordinate care across RNs, LPNs, therapists, social workers and physicians, ensuring plans meet goals and are implemented effectively.
You will supervise LPNs and Home Health Aides, educate patients and caregivers, and maintain documentation in the EMR while adhering to CMS/state guidelines and OASIS requirements where applicable.
