Role:
RN Home Health provides patient evaluations, routine visits, and develops personalized care plans in a home setting.
Responsibilities:Conducts assessments, manages patient needs, collaborates with interdisciplinary teams, and performs nursing procedures. Develops and reviews care plans, communicates with physicians, and educates patients and caregivers to promote self‑care. Ensures compliance with regulatory guidelines and accurately documents care using EMR systems. Participates in performance improvement activities and supports staff education. Maintains communication with team members and reports patient status changes.
Qualifications & Skills:Licensed as a registered nurse in Georgia, with at least 1 year of medical-surgical or home health experience, and strong critical care skills. Certifications include RN license, BLS, and driver’s license with insurance. Proficient in computer systems, including MS Office;
Epic and 3M experience preferred. Ability to coordinate care, communicate effectively, and adhere to regulations.
Meets productivity standards, participates in training, and performs other duties as assigned, including weekend and call rotations. Upholds organizational policies and standards of conduct.
