The OpportunityWe are currently seeking a dedicated Admissions Nurse - RN/LPN
What You Will Do- Conduct comprehensive assessments of residents' medical needs and develop individualized care plans
- Monitor residents' progress and make necessary adjustments to their care plans
- Provide education and support to residents and their families regarding their medical conditions and treatment options
- Collaborate with insurance providers to obtain necessary authorizations for medical procedures or treatments
- Maintain accurate and up-to-date documentation of resident interactions and care plans
- Duties as assigned
- Active RN/LPN license in good standing in the state of practice.
- Experience in case management or care coordination is preferred but not required
