Job Responsibilities
- Utilize the nursing process effectively to deliver high-quality patient care.
- Assess patients' learning needs and implement tailored teaching strategies for patients, families, and interdisciplinary groups.
- Communicate clearly, delegate tasks appropriately, and manage nursing team resources, acting as a leader within the interdisciplinary team.
- Leverage data and knowledge to evaluate performance and inspire changes for optimal patient outcomes.
- Enhance customer satisfaction and employee morale while contributing to the nursing profession's improvement.
- Adapt communication and actions to meet the diverse needs of specific patient populations, ensuring respect for privacy and understanding of services provided.
- Perform additional related duties as required to support patient care and team functionality.

Physical and Environmental DemandsThis role requires a considerable physical effort, including the ability to exert force (50 to 100 pounds occasionally, 25 to 50 pounds frequently, and 10 to 20 pounds consistently). Physical demands exceed those for sedentary work; this position involves active patient care and may require moving objects and standing or walking significant distances.
Exposure to blood, body fluids, and communicable diseases is a routine aspect of this role, as is the potential risk associated with hazardous materials and medications.
Requirements- Current RN license in the applicable state or a Compact License.
- Minimum of 1 year of experience, 2 years preferred.
- Local and travel candidates are encouraged to apply.
- Education:
RN degree required; an associate's or bachelor's degree in nursing is preferred. - Valid Driver's License and Auto Insurance required.
- Minimum 1 year of home health experience.
- Experience with HCHB EMR and OASIS start of care is necessary.
- Current registered nurse (RN) license in the state of practice.
- Current Basic Life Support (BLS) certification from the American Heart Association.
- Proficient in computer use, software applications, and web-based tools.
- Excellent verbal and written communication skills, with the ability to present information clearly and professionally.
- Strong interpersonal skills and effective organizational abilities.
- Self-directed with proficient time management.
- Ability to demonstrate sound judgment.
