Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Dierksen Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community.
Monday – Friday, 8 a.m. – 4:30 p.m. Occasional on‑call required.

- Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch that showcases our commitment to exceptional service.
- Partner closely with the Clinical Director to assess eligibility and suitability of clients for our specialized hospice services, guiding us to provide the right care at the right time.
- Provide direct comfort and support to patients and families, ensuring their comfort and quality of life remain paramount.
- Apply nursing skills by delivering top‑notch care grounded in proven principles and techniques to ease pain and bring solace to those under our care.
- Empower patients and families through education, encouraging active participation in personalized care plans that align with their goals.
- Share the responsibility of call duties, demonstrating our commitment to being there when it matters most.
- Hold a current, unencumbered Registered Nurse license.
- Minimum one (1) year of experience as an RN in a medical‑surgical/acute care setting (hospice experience preferred).
- Computer proficiency in typing and various programs, including experience with EMR systems.
- Current CPR certification, if required by state.
- Valid driver’s license, vehicle insurance, and reliable transportation.
$72,000 – $77,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
