Summary of Position:

Provides support services, provides information, education and support for clients, promotes wellness, recovery and skill building to sustain better health. Provides prevention training for managing stress and problems, making healthy lifestyle choices, recognizing and responding to relapse, managing crisis and achieving and sustaining recovery. Assists clients with personal recovery goals, helps clients develop natural support systems, and coaches clients to advocate for themselves within their healthcare systems.

Implements evidence-informed wellness interventions.

Essential Duties & Responsibilities:

  • Provides group or individual training/interventions focusing on improvement of the client's functioning and health-related issues.
  • Assists clients with developing their Wellness Recovery Action Plan (WRAP). Provides wellness consultation.
  • Empowers clients to self-manage chronic health conditions and solidify wellness goals.
  • Links clients to community-based social support/services and develop collaborative relationships with other departments, services, and community health care agencies to facilitate and support wellness and quality care.
  • Provides ongoing case management services for assigned service recipients.
  • Provides transportation assistance to help clients achieve and maintain wellness goals and initiatives.
  • Conducts intake for recipient entering services.
  • Assists service recipient in making discharge plans and provide follow up services.

Knowledge, Skills & Abilities:

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Knowledge of role and function of various community resources.
  • Knowledge of and ability to instruct assigned clients in basic independent living skills.
  • Skill in customer service/experience techniques.
  • Skill in problem-solving.

Qualifications:

Education Level:

Candidate must be a minimum of 21 years of age per funding requirements. Associates degree required. Bachelor's degree in human services preferred.

Years of

Experience:

Two years of relevant experience working in the human services field; as well as experience participating in their own personal recovery process in order to care for self or family member. SUPR funded positions specifically require 1 year of recovery to be qualified.

Required Certification/Licensure:

WRAP facilitator with in twelve (12) months of accepting position is preferred.

Physical Requirements:

Standing - 20%

Lifting - occasional

Bending - occasional

Pay Range:

$16.50--$23.10
