Marion VA HCS Community Living Center Admissions Coordinator
The Marion VA HCS Community Living Center (CLC) Admissions Coordinator is responsible for overseeing and coordinating all aspects of the admission process for residents, ensuring compliance with facility policies, legal requirements, and quality standards, while providing clear communication and support to prospective residents and their families. Core duties include:
- Admission Process Management:
Serve as the primary point of contact for prospective residents, their families, and referral sources. Conduct initial screenings, gather necessary documentation, and ensure all admission requirements are met. - Referral Coordination:
Oversee external and internal admission referrals, monitor bed availability, and coordinate transitions between care settings. - Assessment & Planning:
Assist with patient planning assessments, identify barriers to admission, and develop individualized care plans in collaboration with nursing staff and interdisciplinary teams. - Data & Reporting:
Compile and maintain accurate admission, discharge, and transfer data for internal reporting, quality improvement initiatives, and regulatory compliance. - Policy & Procedure Compliance:
Ensure all admission activities comply with facility guidelines, state regulations, and federal health care standards. - Communication &
Education:
Provide health education, counseling, and information to prospective residents and families; act as a liaison between the CLC and external agencies, including government bodies and community resources. - Quality Improvement:
Lead or participate in projects to improve admission processes. - Collaboration:
Work closely with nursing leadership, restorative care coordinators, and other staff to ensure smooth transitions and optimal resident outcomes. - Provide support for RAC/MDS positions:
Required as a function of this role is cross-training and shared understanding of the RAI/MDS Assessments and Care Area assessments, completion and updating of care plans and participation in completion, when needed of the MDS assessment process. - This position potentially requires flexibility in schedule and assignments as this is a 24/7 facility. The incumbent may be required to rotate tours and duties during low/high census or complete administrative tasks established by the immediate supervisor. Not all duties may be listed so other duties may be assigned.
In summary, the CLC Admissions Coordinator ensures that residents are admitted efficiently, safely, and in compliance with all standards, while fostering positive relationships with families and stakeholders, and contributing to the overall quality of care at the Community Living Center.

Pay:
