Job Title
The role of the LOCC RN is to establish, promote, and monitor seamless care for Veterans. The incumbent will act as a resource to the medical center staff, contracted, and networking facilities.
The LOCC RN executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care within the VHA Community Care Program. This functional role includes organizing and coordinating the clinical components for the unauthorized/authorized care of Community Care services in response to individual healthcare needs along the illness and care continuum and in multiple settings. Although this position is non-supervisory, it requires the incumbent to exercise clinical leadership in coordinating care across multiple disciplines, facilities, and levels of the organization.
The incumbent is responsible for clinical case management activities necessary to coordinate Veteran care in the community and meet the Medical Center's utilization goals in a manner that successfully integrates community care with past and future VA care. The incumbent works collaboratively with all clinical services across the Medical Center, and with the administrative staff within the LOCC, to ensure coordinated, timely, cost-efficient care is provided to the Veteran.
The LOCC RN frequently acts on behalf of the Medical Center and with other VISN facilities to determine availability of clinical services and ensure proper transfer of patient care. The incumbent participates in collaborative case management planning, utilization review, discharge planning, and other related case management activities, including the review of outside medical services based on established guidelines and medical necessities.
The incumbent communicates directly with Veterans and their families to explain community care processes, benefits, and next steps, ensuring clear understanding and continuity of care throughout the referral and treatment process.
The incumbent is responsible for utilizing multiple computer program systems efficiently to coordinate Veteran care, and for maintaining timely, accurate documentation of all case management activities, communications, and clinical decisions in accordance with VHA policy.
