Traveling Veteran Coordinator (TVC) Registered Nurse (RN)
The Traveling Veteran Coordinator (TVC) Registered Nurse (RN) is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care. The nurse demonstrates proficient practice and decision-making, deliberate planning, and critical thinking skills. The TVC is assigned to Veterans requiring complex care coordination or specialized case management services. The TVC serves as the primary point of contact for these Veterans and their families or caregivers and communicates with the health care team while leading complex care coordination and case management efforts.
The TVC duties are multifaceted and may be divided between RN functions of the TVC Program which includes consult management, care management and coordination of benefits. The TVC serves as a liaison for the Veteran between VA facilities and is responsible for managing the care of the Veteran throughout the process. The TVC supports the medical center's mission to improve timely access to care.
The TVC will function at their highest level of role performance and align high-quality care with key performance indicators to ensure business practice standards meet patient care needs. The TVC will work collaboratively as a member of the interdisciplinary team, making caring for the Veteran their priority while in the work setting, and demonstrate customer service principles in all aspects of work.

Pay:
Competitive salary, regular salary increases, potential for performance awards
