About the Role

The ACT Case Coordinator provides comprehensive, community-based mental health services to individuals with severe and persistent mental illness. Working closely with a multidisciplinary ACT Team, you will help clients access the supports they need to improve stability, independence, and overall quality of life. This is a hands-on, community based position, ideal for professionals who value relationship-building, flexibility, and recovery-oriented care.

• Bachelor's degree in social work, psychology, counseling, or a related field.
• Minimum of 2 years of experience in mental health case management or a related field. Experience working with individuals with severe and persistent mental illness preferred.
• Valid driver's license, auto insurance, and reliable vehicle
• Ability to work flexible hours, including evenings and weekends, as needed.
• CPR and CPI certification preferred.
