About the Role
Practice nursing with autonomy, time, and patient connection to make a meaningful impact every day. As a Home Health Registered Nurse, you will deliver comprehensive skilled nursing care to patients in their homes, supporting recovery, chronic disease management, and overall health goals while helping individuals remain safely in the comfort of their own environment. You will manage a caseload from admission through discharge, conducting assessments, developing individualized care plans, coordinating services with physicians and an interdisciplinary team, and making informed clinical decisions to support patient outcomes and independence.

This is a high‑travel position requiring daily travel within the service area to provide care in homes, assisted living facilities, or nursing homes.
Hours- Monday to Friday, 8 a.m. – 5 p.m., with weekend rotation
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
- Current State License as a Registered Nurse
- Minimum 1 year of experience as a licensed RN in a clinical care setting
- Reliable transportation, valid driver’s license, and valid automobile insurance coverage
- Willingness to travel daily for home visits within the service area
- Bachelor of Science in Nursing (BSN)
- Experience in a home health environment
- OASIS certification or willingness to obtain
- Strong clinical assessment and critical thinking skills
- Proficiency in OASIS documentation and care planning
- Excellent time management and organizational abilities
- Effective communication and teaching skills
- Ability to work independently with minimal supervision
- Comfort working in diverse home and facility environments
- Strong clinical judgment and decision-making capabilities
- Understanding of Medicare, Medicaid, and third‑party payor requirements
