Compensation:
Competitive; based on experience

We are seeking a mission-driven Family Medicine Physician to join a Federally Qualified Health Center (FQHC) providing comprehensive, patient-centered primary care to individuals and families across the lifespan. This role focuses on delivering high-quality, culturally competent care to medically underserved and vulnerable populations in a collaborative, community-based setting.
Key Responsibilities- Provide comprehensive primary care services to patients of all ages, from pediatric to geriatric populations
- Diagnose and treat acute and chronic medical conditions
- Manage chronic diseases such as diabetes, hypertension, asthma, and cardiovascular disease
- Deliver preventive care services, including screenings, immunizations, and health education
- Perform in-office procedures such as suturing, skin biopsies, and joint injections
- Provide women's health services, including Pap smears and contraception counseling
- Order and interpret laboratory and diagnostic testing
- Prescribe medications and monitor treatment plans
- Participate in quality improvement initiatives and performance metric reporting (e.g., UDS measures)
- Support population health strategies for high-risk and medically complex patients
- Collaborate with care coordinators, behavioral health providers, and case managers
- Promote preventive care and health education within the community
- Practice within a Patient-Centered Medical Home (PCMH) model
- Coordinate care with specialists, hospitals, and community resources
- Supervise or collaborate with nurse practitioners, physician assistants, and medical residents as applicable
- Maintain accurate and timely documentation in the electronic health record (EHR)
- Adhere to FQHC regulations and HRSA compliance requirements
- Support sliding fee scale policies and access-to-care initiatives
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
- Completion of an accredited Family Medicine residency program
- Board Certified or Board Eligible in Family Medicine
- Active, unrestricted state medical license
- DEA registration
- Current BLS and ACLS certification
