Job Overview
Gentiva is hiring a Registered Nurse (RN) for Hospice & Palliative Care in Marion, Ohio. The position is ongoing and full‑time staff.
Responsibilities- Serve as the primary point of contact for patients and families, guiding them through the hospice journey.
- Deliver skilled nursing care and document care based on individualized plans, whether in patients’ homes, assisted living facilities, or other settings.
- Perform initial and ongoing assessments, monitor patient condition, and adjust care plans accordingly.
- Collaborate with interdisciplinary team members—physicians, LPNs, CNAs, social workers, chaplains—to develop and refine patient care plans.
- Provide emotional, physical, and spiritual support to patients and loved ones, including education and bereavement guidance.
- Teach caregivers safe and confident self‑care practices, offering practical training.
- Communicate patient condition updates to the care team to ensure timely interventions.
- Maintain accurate, up‑to‑date clinical documentation and adhere to infection control and compliance standards.
- Support quality improvement and scheduling initiatives to ensure consistent, compassionate care.
- Help patients and families navigate medication administration, hospice philosophy, symptom and pain management, and end‑of‑life processes.
Education: Registered Nurse (RN) license required.
Experience: Minimum one year as an RN in acute care, oncology, long‑term care, hospice, or home health. Hospice or palliative care experience preferred.
Licensing & Certifications:
- Current state nursing license.
- CPR certification.
- Valid driver’s license and automobile insurance.
Specialized Knowledge & Skills:
- Knowledge of terminally ill patients and hospice care principles.
- Proficiency with nursing assessment tools and documentation systems.
- Strong communication and interpersonal skills across all socioeconomic backgrounds.
- Understanding of managed care principles, Medicare/Medicaid regulations, and compliance standards.
- Clinical excellence and sound judgment.

- Interdisciplinary collaboration.
- Accountability and regulatory compliance.

- Timeliness of visits and documentation.

- Competitive Salary: $63,370 – $74,505 per year.
