Provide mental health and substance abuse liaison services to individuals in state or private psychiatric and detoxification facilities. Make contacts with hospital staff, treatment teams and consumers in order to coordinate discharge planning. Assist with the tracking of facility admissions and discharges. Assist with the management of LIPOS funding, hospital transfers and Emergency Services insurance information and scan attachments into Credible.
ESSENTIALFUNCTIONS
- Coordinate hospital liaison services with community based agencies and programs to assure appropriate treatment and follow-up. Follow established discharge protocols involving barriers to discharge, needs upon discharge, coordination of transportation and initial referrals.
- Manage LIPOS funds, documents for Emergency Services and state facility transfer requests.
- Write consumer safety plans and maintain documentation on secure site.
- Provide patient information in response to state surveys, individualized service plans, placement options, and resource alternatives, as well as providing consultation in order to respond to family needs of the long term mentally ill.
- Attend meetings for a variety of purposes, such as quarterly liaison meetings, treatment team meetings when needed, and daytime crisis meetings in order to improve the service delivery system.
- Maintain recordkeeping and data collection systems appropriate to the needs of consumers served in accordance with regulatory standards. This involves opening SWVMHI admissions, forwarding progress notes and discharge summaries to appropriate centers, completing the Ready for Discharge forms, follow-up on Community Resource Pharmacy referrals, and provide atypical medication forms to appropriate center nursing staff.
- Make initial referrals for discharged consumers and coordinate transportation and visits to private providers.
- Function as a coordinator of service delivery to ensure that consumers/family members receive effective treatment.
- Good oral and written communication skills
- Knowledge of substance abuse, mental retardation and mental health treatment modalities and counseling approaches
- Knowledge of DSM V criteria
- Knowledge and skill regarding needs assessment, program design, implementation and evaluation.
- Knowledge of mental health and substance abuse confidentiality guidelines.
Bachelor's Degree in human services field and experience working in the mental health field.
Valid Virginia Driver's License and copy of DMV safe driving record may be required.
