Contract duration: 13 weeks

Hours:

40 per week (8 hours, days)

Schedule:

Monday – Friday, 8 AM – 5 PM

Start date:

ASAP

Pay rate: $2,635.00 – $2,715.00 per week (subject to confirmation)

Territories covered:
Milford, Medway, Hopedale, Franklin, Bellingham, Hopedale, Mendon, Blackstone, Charlton, Oxford, Sutton, Northbridge, Millvyle, Uxbridge, Douglas, Oxford, Webster, Dudley, Charlton, Spencer, and Leicester
Monthly travel requirement: 50–200 miles
Weekly productivity unit expectation: 30
EMR system: HCHB

• Physical Therapist, Home Health – 1 year experience
• License: PT‑MASSN required
• Date of Birth required
• 1 reference in entire work history
