Reliable transportation, current driver's license, and agency-required liability insurance

Description:

Home visits will be required.
Periodic office meetings will be scheduled.
The average number of patients per day is 5-6.
Contractors will be assigned between 5-10 patients per day that equate to roughly 6-8 points.
Each type of visit and activity (documentation, travel, etc.) holds a different point value.
Types of cases include Start of Care and Follow Ups.
The contractor must be willing to travel up to 75 miles from the designated clinic.
Mileage will be reimbursed at the IRS Standard rate.
Compensation is provided from one patient home to another.
The commute to and from the first and last patient is not billable.
Weekend requirements vary per location and profession and should be discussed during the interview.
On-call requirements may vary per job location and should be addressed during the interview.
The contractor should be aware that this is a home health assignment.
Daily responsibilities include driving to patients' homes and the clinic for weekly meetings.
The contractor must be comfortable with longer drives between patients.
All documentation must be completed within 24 hours of the appointment with the patient.
Delayed or incomplete documentation may impact billing and follow-up patient care.
There are no guaranteed hours with this contract.
The contractor must hit weekly productivity points of 30, which equates to approximately 40 hours per week.
If not receiving enough patients to meet financial needs, the contractor may have the opportunity to float to another nearby clinic to pick up patients.
