Home Health (Visits) Licensed Practical Nurse

We are looking for a full‑time Licensed Practical Nurse to perform home health visits, providing exceptional medical care to adult and geriatric clients in Marlborough, Northborough, Southborough, Framingham, Natick, and the surrounding towns. Prior clinical experience as an LPN is required.

• Making home visits to clients living in designated territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures.
• Monitoring clients' conditions and reporting changes to the Clinical Manager and Client Services Manager.
• Following up on, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.
• Case management and coordination.
• Accurately documenting observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a touch‑pad tablet.

• A current license as a Licensed Practice Nurse in the state of Massachusetts.
• Graduation from an accredited and approved nursing program.
