Job DescriptionWe are seeking a compassionate and dedicated Palliative Care Nurse Practitioner on a Per Diem (PRN) basis to join our growing healthcare team in Marlborough, Mass.

Compensation and Benefits- Earning potential: $115,000–$200,000 (salary based on experience + easily achieved monthly bonuses)

- Autonomy with support to deliver independent care with access to physician‑level consults
- Deliver person‑centered care using evidence‑based practices
- Collaborate with interdisciplinary facility teams
- Conduct assessments, develop treatment plans, and manage follow‑up care
- Provide ongoing follow‑up and chronic care management in Skilled Nursing Facilities (SNFs)
- Educate patients, families, and staff
- Complete timely documentation (within 24 hours)
- Compliance with facility, state and federal regulations/protocols
- Active license as a Nurse Practitioner (NP)
- Minimum 2 years of post‑certification experience
- Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred
- Proficient with EHRs and clinical technology
- Strong communicator with a collaborative mindset
