Activities Assistant

Reports to : Activity Director

• High School Diploma or equivalent necessary
• Individualized skills in areas such as crafts, music, education or physical activities helpful
• Friendly, outgoing personality, with a warm, caring and patient attitude
• Willingness to work as part of a team

• Assists in planning and implementation of organized program for recreation
• Provides a planned resident‑specific, goal‑directed recreation program for assigned residents/patients; facilitates small group activities as assigned by the director
• Ensures that all charted recreation progress notes are informative and descriptive of the services provided and indicate the resident/patient’s response to the service; assists in developing a written comprehensive plan of care for each resident/patient recreation program that identifies the problems/needs and the goals to be accomplished; follows the established plan of care in providing the daily recreation program
• Involves the resident/patient and family members in the assessment and care planning process when possible
• Transport residents/patients to and from recreation programs
