Job Description: The Care Coordinator will work from our Marlton, New Jersey office. The role ensures high‑quality care services by collaborating with clinicians, staff, and patients to meet health goals and maintain open communication. The coordinator will handle patients’ immediate needs with compassion, organization, and efficiency.
Key Responsibilities- Frequent contact with patients to provide support and manage compliance within care‑management programs.
- Document all client communications accurately, whether verbal or written.
- Respond to primary care clinician or designee communications regarding emergent or life‑threatening patient needs, ensuring complete and accurate care plans.
- Collaborate with cross‑organizational teams to meet patient, family, and partner facility needs.
- Proficiency with technology, including secure email, telephone systems, and electronic medical records.
- Adhere to documentation protocols and best practices for daily work logs and internal communications.
- Demonstrate excellent customer‑service skills evidenced by positive feedback from customers and patients.
- Support the department positively, contributing constructively to the team.
- Comfortable speaking on the phone for large amounts of the day.
- Compassionate and empathetic toward patients, consistently providing exceptional customer service.
- Accurate note‑taking to document each task in a timely manner.
- Ability to multitask between patients and work streams while remaining organized and efficient.
- Thrives in a fast‑paced environment.
- Proficient with computer applications, including Outlook and other Microsoft Office programs.
- Ability to pass a drug screen and background check, and demonstrate a positive attitude, adaptability, teamwork, and willingness to learn continuously.
- Nice to have: Knowledge of basic healthcare terms, conditions, roles, and care principles.
