Summary
Responsible for assessing, diagnosing, planning, implementing, and evaluating patient care through outcome identification and evidence‑based practice for patients in their homes. Coordinates patient care and communicates and collaborates extensively with other healthcare team members, patients, families and significant others to develop the plan of care. Supervises the Home Health Aide in the home. Promotes delivery of quality care and efficient utilization of agency resources.
Compensationand Scheduling
- Participants will receive a 30% premium on top of their hourly rate.
- This position will require two 10‑hour weekend shifts.
- Eligible for 24 hours of paid time off per quarter.
- Benefits become eligible on the 1st of the month post‑hire.
- One holiday required.
- No on‑call required.
- Conduct comprehensive patient assessments incorporating psychosocial and physiological status, health behaviors, compliance with therapeutic regimes and technical procedures.
- Coordinate plan of treatment with the physician through timely and appropriate communication.
- Administer, monitor, and document therapeutic interventions and regimes.
- Demonstrate complete knowledge of established standards for documentation, use of point‑of‑service device, and time frames for completion.
- Educate patient and family/significant other regarding disease process, individual care needs, wellness, medications, safety issues, etc.
- Collaborate with other members of the healthcare team to establish the plan of care.
- Maintain standard precautions and patient safety principles.
- Identify and participate in performance improvement activities.
- Maintain professional and departmental level competencies.
Must have a valid NJ RN License. Approved licenses must be presented and maintained in accordance with the State of New Jersey Licensing Board.
1–2 years recent work experience in a hospital (e.g., Med/Surg, E.R., OB/GYN, Pediatrics, MICU, CCU) and/or one year recent community health experience.
Broad knowledge of nursing practice and ability to apply specific skills in the home health setting.
Required EducationBaccalaureate degree preferred.
SalarySalary: $44.57 – $60.44 per hour
