Clinical Nurse Specialist (JNESO)
Responsible for assessing, diagnosing, planning, implementing, and evaluating patient care needs with selected disorders through outcome identification and evidence-based practice. Coordinates patient care and communicates and collaborates extensively with healthcare teams, patients and/or patient's family or significant other(s). Maintains and demonstrates high standards of professional ethics.
Position Responsibilities:
- Performs a gastrointestinal, genitourinary, integumentary or cardiac assessment of the patient, leading to development, implementation and evaluation of a plan of care
- Administers, monitors and documents therapeutic interventions and regimes related to wound, ostomy, continence or heart failure
- Effectively manages rapidly changing situations
- Participate in clinical decision-making
- Promptly identify and address relevant changes in clinical status to prevent hospitalizations
- Educates patient and family/responsible party regarding disease process, individual care needs, wellness, safety issues, etc.
- Provide education and counseling to patients and/or caregivers Diet restrictions, Medication review, Heart Failure symptoms When to report change in symptoms to nurse/provider
- Delegates and monitors care rendered by other members of nursing department and healthcare team
- Demonstrates standard precautions and patient safety principles in practice
- Participates in orientation, education and development of healthcare team
- Identifies and participates in performance improvement activities
- Maintains professional and departmental level competencies
- Collaborate with multidisciplinary team to achieve highest clinical quality Review of patient cases and implementation of evidenced based practice to affect patient outcomes
- Develops and presents recommendations to nursing leadership for best practice that supports nursing excellence
Position Qualifications
Required:
Required Experience:
- WOCN:
Minimum of 2-3 years of nursing experience. Home Health experience preferred. - Heart Failure:
Minimum of 2-3 years of acute care nursing experience in cardiac care, telemetry or ICU
Required
Education:
- Graduate of an accredited nursing program.
- Baccalaureate Degree preferred.
Training / Certification / Licensure:
- Current State of New Jersey nursing license to practice as a registered nurse.
- Current CPR certification and BLS.
- Must have current proof of valid driver's license and auto insurance.
- Valid New Jersey RN license
- Specialty Certification, CPR certified WOCN: WOC (ET) nurse (CWOCN) by the Wound, Ostomy and Continence Nursing Certification Board or presently enrolled in WOCN accredited ET program with certification within 12 months of hire.
- Certification in Heart Failure from an accredited program preferred
