Hospice Registered Nurse
The Hospice Registered Nurse provides compassionate, high-quality care to terminally ill patients in their home or defined living setting. This role is responsible for assessing patient needs, developing and implementing care plans, administering treatments and medications, and supporting patients and families throughout the end-of-life journey. As a key member of the interdisciplinary care team, the Hospice RN collaborates closely with physicians, intake staff, and referral sources to ensure coordinated, patient-centered care that promotes comfort, dignity, and peace.
This position requires statewide travel.
Key Responsibilities:
- Assess, plan, implement, document, and evaluate nursing care for hospice patients in the home setting.
- Monitor patient condition, identify changes, and respond appropriately through triage and clinical intervention.
- Administer medications and treatments according to the established plan of care to promote comfort and symptom relief.
- Provide education, emotional support, and guidance to patients and families regarding symptom management, the dying process, and end-of-life care.
- Perform wound care, treatments, procedures, and other nursing interventions as needed.
- Observe, document, and report changes in patients' physical, emotional, spiritual, and social conditions to the physician and interdisciplinary care team.
- Conduct pain assessments during each visit and implement appropriate interventions to meet patient comfort goals.
- Collaborate with physicians and interdisciplinary team members to coordinate care and secure appropriate services, supplies, equipment, and referrals.
- Support patients and families during crisis situations and provide post-death care, including documentation and communication with designated contacts when needed.
- Participate in on-call, weekend, and holiday coverage as assigned.
- Maintain accurate, timely, and complete clinical documentation in accordance with organizational and regulatory standards.
- Follow all organizational policies and procedures, including HIPAA and patient confidentiality requirements.
- Maintain clinical knowledge of hospice care practices, regulations, and reimbursement guidelines and apply them to patient care.
- Maintain nursing supplies and a safe, clean working environment.
- Participate in QAPI initiatives, interdisciplinary team meetings, and other assigned duties or projects.
Qualifications:
- Minimum of 2–3 years of hospice nursing experience required; home care experience preferred.
- Strong assessment skills and sound clinical judgment.
- Knowledge of the physical, psychosocial, and spiritual needs of terminally ill patients and their caregivers.
- Graduate of an accredited nursing program; BSN preferred.
- Current New Jersey Registered Nurse license in good standing.
- Current CPR and BLS certification.
- Valid driver's license and current auto insurance required.
