The RoleWe're looking for a registered nurse to support adults in their homes and communities. This is a hybrid, per diem position: most of the work, documentation, coordination, and care planning is done remotely. In-person time is reserved for what truly requires it: patient evaluations, assessments, and CHHA determinations. All clinicians are expected to participate in the firm's accreditation activities and surveys to maintain our quality standards.
What You'll Do- Conduct comprehensive health and psychosocial evaluations, and monitor each patient's condition.
- Coordinate total patient care—health promotion, prevention, service coordination, and teaching.
- Determine the need for and frequency of CHHA services based on patient and home assessments.
- Develop and implement individualized nursing care plans; evaluate their effectiveness on an ongoing basis.
- Complete admission, transfer, re-certification, resumption-of-care, and discharge documentation.
- Submit clinical and progress notes weekly, plus a daily tally of patient visits.
- Consult with attending physicians on care plan changes; loop in your supervisor as needed.
- Present records to the Clinical Record Review Committee and participate in case conferences.
- Explain nursing and firm services to patients and families as part of care planning.
- Participate in firm accreditation activities and surveys.
- Active NJ Registered Nurse license in good standing.
- Mid-level clinical experience; home care or I/DD experience preferred.
- Comfort working independently in a remote-first workflow, with reliable transportation for in-person visits.
- Strong documentation habits and clear communication with patients, families, and physicians.
