Senior Licensed Practical Nurse (LPN)

Lakewood Family Medicine Clinic – Full-Time – 1.0 FTE or 80 hours/pp

Schedule:

5 x 8-hour Day Shifts, 7:30 a.m.–4:00 p.m. or 8:00 a.m.–4:30 p.m.

Sign-On Bonus: $7,500 available to qualified applicants

• Collect and document patient intake data; maintain accurate and complete patient records.
• Provide patient support that reflects consideration of emotional, physical, social, cultural, religious and economic influences.
• Assist with routine clinic support activities.
• Advocate for the physical and emotional well‑being of the patient.
• Maintain an appropriate, safe and therapeutic environment per clinic policy.
• Coordinate nursing staff to facilitate accessibility and continuity of care.
• Coordinate physician and office staff requests regarding patient activities and needs.

• State of Michigan LPN license.
• Graduation from an LPN or LVN program.
• Basic Life Support Health Care Provider (BLS-HCP) certification.
• At least six (6) years of clinical experience.
