Registered Nurse (RN) Care Manager - Lakewood Family Medicine
Job Type: Full-Time | 0.8 FTE or 64 hours per pay period | Exempt/Salaried
Schedule:
8 x 8‑hour day shifts per pay period, flexible start times
We are looking for a compassionate and skilled RN to lead care management at Lakewood Family Medicine in Harvey, Michigan. The RN Care Manager will build meaningful patient relationships, coordinate care across the healthcare continuum, and help patients achieve better health outcomes as part of a collaborative, patient‑centered team that values compassion, teamwork, and work‑life balance.
How you’ll contribute- Identify the targeted population and implement clinical interventions and protocols.
- Collaborate with PCP, patients, and members of the health‑care team including continuum‑of‑care settings and community resources.
- Provide self‑management support and involve patient/family as active members of the team approach.
- Facilitate close monitoring of high‑risk patients to prevent and/or intervene early during acute exacerbations.
- Coordinate care between PCP and acute‑care hospitals, specialists, and post‑acute‑care services.

Applicants must hold a current Michigan RN license and possess at least an associate’s degree from an accredited nursing school (a bachelor’s preferred).
- Basic Life Support Health‑Care Provider (BLS‑HCP)
- 2 years in adult medicine and pediatric patients
