As an RN Care Manager, you'll build meaningful patient relationships, coordinate care across the healthcare continuum, and help patients achieve better health outcomes. • Identifies the targeted population and implements clinical interventions and protocols.
• Collaborates with PCP, patients and members of health care team including continuum of care settings and community resources.
• Provides self‑management support and includes patient/family as active members in the team approach.
• Facilitates close monitoring of high‑risk patients to prevent and/or intervene early during acute exacerbations.
• Coordinates care between PCP and acute care hospitals, specialists and post‑acute care services.

Applicants should have a current Michigan RN license and possess an associate's degree from an accredited nursing school (bachelor’s preferred).

• Basic Life Support Health Care Provider (BLS‑HCP)
• 2 years in adult medicine and pediatric patients
