Job Overview
Registered Nurse (RN) Care Manager – Lakewood Family Medicine (Lakewood Family Medicine, Harvey, Michigan). Full-Time | 0.8 FTE or 64 hours per person‑per‑week. Exempt/Salaried.
Schedule:
8 x 8‑hour day shifts per week with flexible start times.

As a Care Manager RN you will build patient relationships, coordinate care across the continuum, and help patients achieve better health outcomes. You’ll work in a collaborative, patient‑centered team that values compassion, teamwork, and work‑life balance.
Responsibilities- Identify the targeted population and implement clinical interventions and protocols.
- Collaborate with PCP, patients, and members of the health‑care team including continuum of care settings and community resources.
- Provide self‑management support and involve patient/family as active members in the team approach.
- Facilitate close monitoring of high‑risk patients to prevent and/or intervene early during acute exacerbations.
- Coordinate care between PCP, acute‑care hospitals, specialists, and post‑acute care services.
- Current Michigan RN license.
- Associate’s degree from an accredited nursing school (Bachelor’s preferred).
- Basic Life Support Health Care Provider (BLS‑HCP).
- Minimum 2years of experience in adult medicine and pediatric patients.
