Are you a compassionate Hospice Nurse looking to make a meaningful impact in patients' lives? Do you excel in providing care in a home setting, ensuring that both patients and their families receive the support they need? If so, we have an exciting opportunity for you!
Join our healthcare partner as a Hospice Registered Nurse, where you will provide direct skilled nursing in the home setting. You will utilize all phases of the nursing process, including assessment, diagnosis, planning, implementation, and evaluation, to deliver exceptional patient care.
Your role will be crucial in coordinating patient and family care across all disciplines to meet their physical, emotional, spiritual, and social needs.
- Provide direct nursing care in the patient's place of residence, adhering to professional standards of care
- Coordinate care with physicians and other healthcare team members to ensure comprehensive support
- Follow the physician-ordered plan of care and obtain additional orders as necessary
- Complete all required documentation in a timely manner
- Maintain a safe environment for patients and their families
- Participate in continuous quality improvement activities
- Collaborate with the team to implement policies and procedures
- Recognize the importance of orienting and building partnerships with healthcare team members
- On-call one full week every month, including evenings and weekends
- Need 2 years experience in Hospice setting
- BLS (AHA) certification required
- Michigan state license required
